click on the requisition
form to open up the pdf file. Once it opens, you can then print
and fill in the form. Along with the tissue blocks or unstained
slides, you can submit your request to NP 3520. You can also fax
the completed form to 205-975-7834 if it is an UAB case. Currently,
we are not able to accept requests electronically. If you don't
